HW: Covers All, Tape 5

I am willing to make the tape labels again for tape 5.
I still have the original Quark files, graphics, fonts, etc.
I even still have the alternate titles that we had discussed
at the time.  Probably even have a new color of paper that
wasn't used on the first four tapes (the USA tape labels were
all sorted by color, with tape I being pink, tape II being
blue, III white, IV orange).

Count me in again.
