BOC: "Burnin' For You / Dr. Music / Flaming Telepaths" EP

I was chatting with Andy Gilham, ex of this list, the other day, and he talking about an old 12" BÖC single with a live "Flaming Telepaths" on the B-side, which I'm guessing must have been this: http://www.discogs.com/Blue-Öyster-Cult-Burnin-For-You-Dr-Music-Flaming-Telepaths/release/1500503

Anyone know if that live version of "Flaming Telepaths" ever saw CD release anywhere? I am not sure I remember an official live "Flaming Telepaths" on CD (though I think I have at least one '70s-era bootleg with it).

Admittedly, discogs.com says it was "Recorded Live At Bonds International Casino New York June 16, 1981", which I think has appeared as the "Tales from the Psychic Wars" bootleg, which does not contain (AFAIK) a "Flaming Telepaths", so I suspect that the recording details that discogs.com give for the live "Dr. Music" on the 12" -- "Recorded Live At Nassau Coliseum New York Oct. 17, 1980" -- are actually the details for the live "Flaming Telepaths".  That's the same show that contributed "Black Blade" to "Extraterrestrial Live" (and was filmed for release as the "Black & Blue" video with Dio-era Sabbath), so it seems plausible that other recorded performances from the show would be kicking around .... But I have not otherwise seen this particular live version of "Flaming Telepaths" surface elsewhere (and it's not on "Black & Blue").
